Visiting Student Consent

This form is for families who plan to have their student shadow classes at Vine Academy while considering Vine for its academic program.

By completing and submitting this form, the parent or legal guardian requests permission for the student to visit campus and understands that the student will be under Vine Academy's care and supervision during the shadow visit. The parent or guardian also agrees that the student and family will follow Vine Academy's rules, safety instructions, and campus expectations.
