WAIVER & RELEASE AGREEMENT

2026/2027

Please read carefully before signing, as this is a release of liability and of certain legal rights.

In consideration for my being permitted to participate in all Vine Academy events, both during Vine hours and off-campus events (such as field trips or leaving campus during Vine hours), I agree to the following Waiver and Release:

1. I agree to observe and obey all posted rules and warnings, and further agree to follow any oral instructions given by Vine Academy of its employees, representatives, or agents.

2. I recognize that there are certain inherent risks associated with the above-described activity. I assume full responsibility for personal injury to myself, my family members, and further release and discharge Vine Academy for injury, loss, or damage arising out of me or my family's use of or presence upon the facilities of Vine Academy and Gilroy Presbyterian Church, whether caused by the fault of myself, my family, Vine Academy, or other third parties.

3. I agree to indemnify and defend Vine Academy against all claims, causes of action, damages, judgments, costs, or expenses, including attorney fees and other litigation costs, which may in any way arise from me or my family's use of or presence upon the facilities of Vine Academy or Gilroy Presbyterian Church.

4. I agree to pay for all damages to the facilities of Vine Academy caused by me or my family's negligent, reckless, or willful acts.

5. Any legal or equitable claim that may arise from participation in the above shall be resolved under California law.

I have read this document and understand it. I further understand that by signing this release, I voluntarily surrender certain legal rights.

Must be signed by all parents/guardians of the minors listed above.
