Read and sign below if you acknowledge and agree to the following:

• I grant Vine Academy permission to use photographs, video recordings, and other digital images of myself and/or my student(s) in any of its publications and promotional materials, including but not limited to printed materials, social media, websites, and other electronic communications, without compensation or additional consideration.

• I understand and agree that all photographs, videos, and digital images become the property of Vine Academy and will not be returned.

• I irrevocably authorize Vine Academy to edit, reproduce, copy, display, publish, distribute, or otherwise use photographs and videos of myself and/or my student(s) for the purpose of promoting and publicizing the school's programs, activities, and mission.

• I waive the right to inspect or approve the finished products or materials, including printed, digital, electronic, or online publications in which my or my student(s)' likeness appears.

• I release, indemnify, and hold harmless Vine Academy, its employees, volunteers, representatives, and agents from any claims, demands, liabilities, damages, or causes of action arising out of or related to the authorized use of photographs, videos, or digital images of myself and/or my student(s) as described above.

• I certify that I have read and fully understand this Photo & Media Release and voluntarily agree to its terms.
